Would You Like To Learn How To Better Manage Your Chronic Condition?
JFS is proud to be an ally bringing information about the MyCD program to seniors across the state. JFS New Mexico staff are trained MyCD workshop leaders and are working to grant access to this invaluable program to our Community Innovations Aging In Place sites.
The goal of the Manage Your Chronic Disease (MyCD) program, a joint venture between the City of Albuquerque Department of Senior Affairs and the New Mexico Veteran’s Affairs Health Care System, is to improve the physical and emotional health of individuals age 50 and older with chronic diseases, and their caregivers, by helping them gain self-confidence in their ability to manage their symptoms and how their health problems affect their lives.
The MyCD Program follows the Chronic Disease Self-Management Program developed by Stanford University’s School of Medicine Division of Family and Community Medicine.

MyCD Chronic Disease Self-Management - Program Description
Ten to 15 participants meet once a week for six weeks. Sessions last for two and half hours with ample break times.
Each week a new topic is discussed and participants help each other set and achieve goals.
This program is FREE.

EnhanceFitness
EnhanceFitness is a low-impact fitness program for adults age 50 and better developed by Senior Services (Seattle, WA).
“Based on solid research and tested at over 100 sites around the country, EnhanceFitness focuses on stretching, flexibility, balance, low impact aerobics, and strength training exercises — everything health professionals say we need to maintain health and function as we grow older.”

A Matter of BalanceThis award winning* program is designed to reduce the fear of falling and increase the activity levels of older adults who have concerns about falls.
This program utilizes volunteer coaches to teach the eight two-hour sessions.
Classes Help Participants Learn To:
View falls and fear of falling as controllable
Set realistic goals for increasing activity
Change their environment to reduce fall risk factors
Promote exercise to increase strength and balance*Awards

Strong Women
Here is some quick info about Strong Women:
Classes are held twice a week for 12 weeks
Increases muscle strength and endurance for women and men of all ages and ability levels
No strength training experience needed!
Earn incentives and a certificate upon completion
The Strong Women Program was developed at Tufts University by Dr. Miriam Nelson.
