OVERVIEW & BENEFITS - NURSE CASE MANAGEMENT

The Nurse Case Management program supports individuals on two different levels: the no-charge service and the private fee-for-service. The no-charge service is non-invasive nursing geared to helping people in their home manage their medications safely and accurately. The nurse also provides health education, screening, and medical advocacy for seniors. The private fee-for-service includes this and also services like accompanying the client to healthcare appointments and visiting different health care facilities to assess situation.

SERVICES

JFS RN Case Manager offers a scope of services, including:
• Medication review and organization
• Physical assessment
• Communication with physician and family to advocate for client
• Referrals to community resources
• Education related to medication and disease process
• Accompanying fee-for-service clients to healthcare appointments and advocacy during inpatient admission situations.

PROGRAM DETAILS

Jewish Family Services of New Mexico provides fee-for-service RN Case Management services in conjunction with the Geriatric Case Management Service. Referrals can be made directly for the RN or by having the Geriatric Case Management make an assessment and refer to the RN as needed.

Our experienced Registered Nurse can visit the home and conduct a physical assessment of the client as well as review their medications and how they are being managed. The nurse will then develop an individualized plan to help organize the medications and will communicate with the physician and family as needed.

Frequency of visits can vary and can be individualized to need for fee-for-service clients. The no-charge nursing visits are up to once a week for a limited duration.
